5. Identity and description of the documentary heritage
5.1 Title of the documentary heritage item or collection
This should be the complete name of the documentary heritage including dates (if they are part of the name) as it is usually known.
5.2 Catalogue or registration details
This should include any registration, inventory, classification and/or call numbers that uniquely identify the documentary heritage.
Example
Journal of Lieut. James Cook kept on board H.M. Barque Endeavour on a voyage round the world, 1768-71.
5.3 Visual documentation, if appropriate (for example, photographs or a video of the documentary heritage)
Where available, any photographs or videos of the documentary heritage should be supplied.

5.4 History/provenance
This should be a summary of its provenance (for example, how and when was the material acquired and integrated into the holdings of the institution).
Example
Purchased by the Australian Government at a Sotheby's sale in London in 1923. From 1868 to 1923 it was in the possession of the Bolckow Family of Marton-in Cleveland, Yorkshire, England.
A bibliographic description of the physical characteristics of the documentary heritage might also be included.

5.5 Bibliography
This should be, where possible, a list of up to three published sources describing the documentary heritage. The preferred style for bibliographic references is described in Elstob, R. (ed) 1988, Style manual for authors, editors and printers, 4th edn, AGPS, Canberra. or its most recent edition.
5.6 Names, qualifications and contact details of up to three independent people or organisations with expert knowledge about the values and provenance of the documentary heritage.
This should be, where possible, a list of the names, qualifications and full contact details of up to three independent people or organisations who could give an informed opinion about the significance and provenance of the documentary heritage. Nominators should obtain the written permission of the named experts prior to completing the nomination form and are required to declare that they have this permission by completing this section of the nomination form.
I assert that the above named referees have given their written permission for their names and contact details to be used in connection with this nomination for the Australian Memory of the World Register in 2006, and their names to appear on the website.
